- Important Reminders:
– Annual Health Forms – If you have not submitted a new health form in the past two months, please do so before September 8th.
– Shot Records – All students must have a complete, updated, shot record (or an official exemption waiver) on file BEFORE your child can start school.
– Vision & Hearing – If your child is 4 years or older, we are required to have a copy of their vision and hearing screening on file.
– Does your child have an allergy? – If your child has an allergy and/or will require a medication, there are required forms we will need on file for your child. Please see below:
